Anyone know if Kaiser in OHIO covers WLS on the individual plan?

I am self employed and I am checking out different companies for health insurance. I plan to have this surgery in the near future. I was wondering if there is anything I should make sure the policy has to ensure that they will cover the surgery without coming right out and asking? Just because a policy has major medical doesn't mean it will cover the WLS does it? Since I was overwieght before I had the ins. could they call it a pre-existing condition? Anyone who knows please help it is greatly appreciated. Thanks
