Does anyone have a sample letter to their insurance Co.that they can share with me?

HELP!...my insurance company needs more medical documentation of previous weight-loss attempts...the only problem is that my old health records were lost when my health clinic closed.I am in the process of writing a personal letter and need sample letters. Can anyone direct me to a good site for sample letters or actually share theirs with me? Thanks in advance!
