Do you have any info on doctors and insurance carriers that cover children?

My daughter (16) and I (50) have decided to do the WLS after attending Dr. Wittgrove's seminar. We are looking for insurance and surgeons who have done the LAP for children. I say she is a child only because of her age. Physically she is a woman.
