what exactly do they consider to be a co-morbidity? trying to get approved!!

I am border line over 100 lbs depending on which table i'm using. I'm a little confused over what they consider co-morbidity to be. I don't have a heart problem just pain, swelling, and fatigue because of my weight. Do you think this and documents of past prescritions taken will be enough to get me approved. And does anyone know of a site of a sample letter? thanks
