Initial insurance letter for approval of a tummy tuck

I just went a saw a plastic surgeon I really like for a tummy tuck. The only problem is that he will not to thru insurance, because he says most of the time tummy tucks are not covered. He said it would be up to me to submit info to the insurance company for predetermination. My question is does anyone have info that I could have to what I need to include to send to the insurance company. For example a sample letter, etc. Any info would be greatly appreciated. Thanks in advance.
