Seeking Other Lap Banded Post Ops Covered By BCBS of Florida

My BMI is 38, I'm 5'5" 227 pounds, but I have high blood pressure, poly cystic ovarian syndrom (infertility issues-insulin resistant)acid reflux...I'm in Wisconsin and our health insurance is though Blue Cross Blue Shield of Florida. Any help would be great!
