Does anyone have sample letter for insurance submission

My nutritionist called me he states he has never submitted letter for RNY surgery to insurance before. Does he only need to submit his eval or should he write a letter to ins company also? My surgeon said copy of his eval would be sufficient. Have you all found this so? I am giving him a list of my attempted diets since I was a teen. Any help would be appreciated. Thanks
