Anyone been denied a TT because of a picture, even though it's medically necessary??

My insurance will approve a abdominoplasty if it is medically necessary. The PS sent the letter of necessity, and now the insurance wants to see pictures. My worry: because I don't "hang" that much, I'm afraid that they won't approve it. Do these companies "compare", or expect the stomach to look a certain way? Is it possible that they could deny my request based on a picture? I want this SO bad, and I would like some encouragment from people "in the know" that a picture will not be the thing that gets me denied. Thanks everyone!
