did anyone get approved with bcbs/fl/ppo/blue choice w/ no recent 12 mth. supr. diet?

I have this ins my 1st. consult is this Thurs. and I called the customer service rep. @ bcbsfl and she told everything I need she said I have to have 12 dr. supervised weight history I told her i have but from only 1995-1996 and she said she thinks it has to b more recent w/ proof of morbid obesity for 2 yrs. i have proof of that but from my obgyn and the hosp. but not from 1 dr.I also have tried all other diets I did write a long list of.DID anyone get appr. w/ them without recent dr. supervised weight loss?Please let me know thank you in advance.AND SHOULD I JUST GO TO MY FIRST CONSULT ANYWAY AND TRY ?
