I need a copy of a referal for consultation letter from PCP.

I was able to get a consultation appt for one week after I called to schedule. My insurance is a PPO and does not require a referal for consultation. I just got a letter yesterday from the surgeons office stating that I will need to ask my PCP for a referal for "consult for obesity". I contacted my Dr.'s office and they said that due to the short notice (consult is on Tuesday) that I can type up a referal letter and take it in for the Dr. to sign. Now I need to know what to put in the letter. If someone has a sample they can send me I would really appreciate it! My email is [email protected].
