Who takes a Medical Card in ILL?

I live in Rockford, ILLinois. I am 480lbs, and i have many many many health issues and meet all the requirements that would be needed for my medical card ( public aid) to get the surgery done. Problem is this, now they tell me that they dont cover the surgery ( Baratric Treatment ceter told me this). Then another clinic said that it does but not the hospital stay. My caseworker has no idea what to do or anything, with over 200 clients i guess not. My doctor wants the surgery done for me and gives me less than 10 years to live without it, But cant find anyone that will take the card to get the surgery done. i'm soo confused. any suggestions? please..
