Appt. with vocational,3-days how should i bring up surgery?I have Dr. referral

I live in Central Fla. i have talked with one person(e-mail) briefly about Voc. Rehab. paying for her surgery, she had less physical problems than I do, bu she lives in the next town from me and it is alot bigger. so my question id all things being equal can they turn me down I met all requirments foe V.R. I guess I am really afraid of a (no) I have been waiting a while to get this second appt. should I call them a head to tell them what I need?
