How long after approval till you gat a date?

My approval date was April 11, 2001, I guess I'm just antsy to get started on my new life. Scared yes, but I had brain surgery 2 years ago and figure this is a piece of cake compared to that. What is the longest you had to wait for a date and can the insurance company unapprove you once you been approve.
