What kind of denials has anyone received from Allmerica?

I am waiting for approval from Allmerica, but I am concerned that they may deny me. I do not have any existing co-morbidities, but I know that I am sure to have them without surgery . (I am 27, with co-morbidities running in my family.) What kind of reasons has Allerica given for not approving surgery?
