EXCLUSION IN POLICY. HOW IS A GOOD WAY TO APPEAL?

I have been denied secondary to an exclusion in policy. States will not cover WLS for any reason, even if medically necessary. I have to get the surgery and can't afford it on my own. How do I go about appealing it? And writting a appeal letter to HR? Thanks Bonnie [email protected]
