Dr Custer/Columbus--initial consultation--What to expect

Initial consultation with Dr Custer soon approaching. What can I expect? I have prepared diet history,5yr previous medical records,bariatric questionaire,patient infomation form. By time of appointment will have results of psych evalulation and will have had ititial sleep evaluation completed and testing scheduled. Anything else I can do to be prepared? Any help appreciated.
