Has anyone had this surgery who employed with Schmidt Baking Co?

Our insurance carrier is BCBS of MD Carefirst under a national plan for Schmidt Baking Company. There is an exclusion in the plan. Has anyone dealt with this and gotten past the exclusion, even though MD now has a law indicating they must insure with medical necessity? Thanks!
