I need to write a letter to my insurance company to help with approval

I have had three doctor supervised diets in the last 6 years. Two of those doctors conviently lost my medical records, so my surgeons office suggested i write a letter to my insurance company, stating how long ive been overweight etc etc any body out there have a format to follow. Any help would be appreciated!!! THANK YOU!!!
