Doctor supervised diet.

Help! I was just denied because they didn't send in any doctor supervised diet history. I have been overweight since I was at least 13. I started on Weight Watchers then and multiple other diets since then. I went to a doctor in June of 2000 because of my diabetes and they put me on a diet for diabetics but I never went back to see him. I didnt like the doctor, but stayed on that diet. Any help Please..
