Should I write a letter to my insurance co. and let them know what all is wrong

I am Borderline on my weight I am 5'1 and weigh 222 right at 100 pounds overweingt, My famly Dr wrote me a letter of recomndation for this surgery BUT should I write a letter also along with my surgon.?
