BCBS of TX approval for WLS hiatal hernia/acid reflux surgery

I wanted my surgeon to perform surgery for my hiatal hernia/acid reflux which is becoming unbearable. He refused because he felt the surgery would fail because of my weight and agreed to submit a letter to BCBS of TX(healthselect through the state of texas) for a wls instead of the "usual" surgery because he has had several patients that have been "cured" by the surgery. However, BCBS excludes surgery for obesity. Does anyone have a referral letter requesting WLS for solely a hernia/acid reflux situation. (The cpt(?) code is not excluded but will be if for obesity.) Thanks.
