I received approval for a consult and two follow ups; does the surgeon have to submit

for approval next? Do I have a consultation and then what? What are the follow ups for. Do you normally have three visits prior to surgery? Then the surgeon requests approval? I don't know how this really works. All I know is I have a copy of the APPROVED: 1 CONSULT/2 FOLLOW UPS that is all it says, and I faxed it to the Surgeons office, and they are supposed to callme to make an appt????
