What's next?

Utilization review committee at my PCPs medical group denied his request for me to be referred for consultation to a gastric surgeon. They say I haven't had enough "conservative management". Recommending a nutritionist (have an appointment in July) and physical therapy (what's this for??) I've already had foot surgery in April because of problem due to weight, podiatrist wants to operate on the other foot now because of another problem due to weight. I can hardly walk. Have developed diabetes now, have GERD, and strong family history of heart disease. What do I do next?
