UMTA trust / US Benefits , Has anyone gotton approval by this ins.?

I am interested in getting surgery. My insurance said I would need to appeal to get surgery. This insurance is self-funded. This is a metal workers union insurance. Has anyone been able to get surgery with this insurance? I live in Oregon but info from another state is also appreciated. Our plan is a PPO but I can see any Dr. I choose. I would like to know what I am up against. Thanks
