what documentations do u first need when u apply to ohip for aprroval for surgery?

have decided to have sugery , have done research on my own but going to talk to dr again about it and wanted to start the application as soon as possible for ohip to cover it . but would like to know what since still in early stages of things what should i have for paperwrok right now . for testing all i have had so far is the sleep lab for sleep apena . if anyone can help i woudl be so gratefull.
