Does anyone have sample letters who were approved

I have lost 150 pounds since last June 23 and I am trying to get BCBS of IL approval for arm reduction, my worst area, breast lift and TT. So far, I'm trying to get necessary info. as was told my PS letter wasn't good enough. Any help would sure be appreciated. Thank you.
