In process of trying to get insurance to pay for a LBL.

In case insurance only pays for a panniculectomy, I will pay the difference for the LBL. My question is, if I run into complications, who pays? Does it depend on the complication? Does it depend on the insurance co (I have Aetna US Healthcare)? Thanks!
