Help,i have been denied ..and i need to write an appeal letter

i have been denied ..Mamsi called my surgens office today with the bad news,my surgery date is 4/06/04 two weeks away .i called the insurance commisioners office today and was told to write an appeal letter......does anyone out there have a good one that you could e-mail to me?
