HAS ANYONE WITH BCBS OF MI/TRADITIONAL HAD BREAST REDUCTION/LIFT COVERED?

Hello Everyone :o) I'm almost 1 year out and would LOVE to have breast reduction and breast lift surgery. I have BCBS of Michigan(traditional) and they won't tell me anything without a code. Has any of you with the same insurance had Breast Reduction and Lift that was covered. If so, what all did you have to provide them with? Thanks a bunch for any help you may have.
