What kind of documentation is needed to get approval for Plastic surgery??

Hi,I am 1 year out, down 151 lbs (YEAH!!) and I am SO going to need a leg lift, butt lift, booby lift and my arms done. What should I be doing NOW in order to help my insuraunce pay for this? And is it reasonable to hope for insuraunce approval in these surgeries?? Thanks for your help!!!
