What do I say to PCP that I've never meet before?

I have an appointment with my PCP and I have never meet them before. It milatary so they change doctor at times. Well the doctor I was used to is gone and I'm nervous. I have fibromyalgia and bad knees, so it's hard for me to get around. That is my reason for the surgery. My bloodpresure is broadline high. I maybe broadline diabitic. Do I ask for all these test or that? Please help me!!! Jody
