I need the recipe to make hot chocolate.

I love the little packs you buy & make, but I'd like to buy the cocoa, sweetener, dry milk, etc.. to make up a container full. The kids love it & it would be easier to add more or less to our liking. I know someone posted the recipe some time back, but I called myself searching, & can't find it. Thanks in advance!
