I would like to know if anyone with Cigna PPO in Va has had trouble getting approval?

Specifically, I have Cigna PPO through my husbands work (Best Buy)when I call Cigna to ask about specifics they told me they approve on a case by case basis. So, I just want to know if anyone else out there from Va has experience dealing with them and what the outcome(s) was/were. Thanks!
