Need advice about Dr. appointment

I have PPO insurance so i don't need a referal from PCP. Which is good because i don't have one. Anyway, since i haven't seen a Dr. for so long my surgeons office told me to go to a Dr. and get checked out so i can get my comorbs documented. My question is what should i ask the Dr. to test for? And after the appointment can i get a copie of their notes and such that day?
