PCP letter: medical clearance and comords?

Sorry for all the questions but I am getting ready for a round of appts and need to construct the PCP letter. I need a letter from my PCP to my surgeon that has my history, comords and clearance. What should the language of the clearance be? that I am in good health? How can that be with so many comords? Just curious how to balance this. Thanks again.
