Can not locate

I am trying to help my sister gather her documentation for insurance. They want all prior diet documentation. Does anyone remember the "Panda Weight Loss Clinics". She used Phen-fen from that clinic, but they have long since closed down, and now she has no proof of that "documented weight loss attempt". Any suggestions would be greatly appriciated.
