I need help getting evaluations done for surgery approval.

Does anyone in Mass. know of a dietician or shrink that i can see for an eval for surgery? The one i saw last month wants me to eat like i already had surgery and keep a journal for a month before she will approve me. The dietician thinks i should just diet, period. I need help! Can anyone offer any suggestions as to what i should do? Thanks for your help.
