Has Anyone Ever been Approved without even knowing it at first?

I was approved to have a Gastric Bypass on June 10th 2002 and the Surgeon had not even sent in the request letter yet. I believe what happened was that my pcp submitted all of my test results along with her request and they approved it that way. The Surgeons office is still trying to figure out how I got approved on the first try so quickly and without the Surgeons letter. Has this happened to anyone else?
