Has any one dealt with Unified Groupe or self funding insurance?

My husband employer has denial my claim for surgery so I'm going to appeal. But the insurance company told me they are self funding what ever that is. So they don't have to go by Indiana laws for insurance. 2000 they passed in legislative a law for obesity, but they don't have to go by it. They go Dept of Labor, and they said thats right. So if any one could help me i would be greatly appreicated. Thanks, Angel Hopeing some day to be my turn.
