Does anyone have a sample letter you wrote your ins company?

Hi..I am seeing my surgeon on Oct1..I am preparing, maybe overly preparing for the appt..I am wondering does anyone have a sample letter that they wrote to send to their insurance company? I have AETNA PPO..I wrote one but it so long..CAn anyone help me?
