Does anyone know if my insurance company cover this surgery?

I have the United Food and Commercial Workers Mid-America Health and Welfare Plan or MAT. It is a union health care plan, Does anyone have any experience with it. I am really wanting this surgery, but I do not know anyone who has used this insurance. Also If there are any suggetions on what insurance companies, that anyone can enrollin, would cover it.
