What are the most important ??s I can ask my surgeon on initial consult?

tomorow is my initial consult & I'm very exicted/nervous. Have a bmi of 36, hypertension, stress urinary incontinence, chronic low back pain and depression. Have about 100 lbs to lose. What type of surgery do I really WANT based on all the information out there? HELP . . .
