Has anyone bought a bluecross plan to have surger,if so which one?

Hi everyone! I in the process of switch from cobra to blue cross and I don't know which one I should purchase. Im thinking about the ppo share1500,but would like to know if someone else out there has the same plan and about how much of the cost did you have to pay.I know sometimes they make you pay more according to your weight. Well I weigh 235 lbs. and I live in California.
