Has anyone used Anthem Blue Cross/Blue Shield of CT(Blue Care Plus)

I need to know if qnyone has had trouble getting approved for surgery through Anthem Blue Cross/Blue Shield. I also think that my PCP is going to give me a hard time about my resolve to go forward with this. Can anyone recommend a good PCP in CT; also a surgeon who will do the lap GBP?
