Have any of you made out a living will?

My surgery is next month. When my husband had throat surgery earlier this year we were asked at check-in time whether or not he had a living will. Have any of you done this? What does it cover, organ donation for instance? If you have a living will where did you go to get started? How far in advance does it need to be done? Does it need to be registered anywhere?
