Help I need a sample of an appeal letter for my HMO

The PCP who suggest I have WLS and would have helped me through the process has left the practice. She explained to my new PCP of my needs and is also willing to help me. Unfortunatelly, the letter she wrote to the my HMO is not strong enough. Is there an example for me to show her? Please HELP.
