What can a person like me do when my insurance carrier is through the goverment

I am on a fixed income and i have health care through a company that is contracted by the State of Ohio. they have told me on the phone that they do approve the surgery but has denied me twice. What can I do to make these People see that even people on a fixed income need the surgery and the help too?
