I was recently approved for surgery but have ?

I recieved a phone call saying that I was approved and giving a surgery date. My question is after the drs. office calls due you recieve something from your ins co. saying how much is your portion or does that come after your surgery with the bill. The reason why I'm asking is because Ive left a relationship and I don't want my ex to get the letter (if any). So basically my question is do you get anything in writing from the ins. co even if you know that youre approved already because the dr. sent the infor to the company and it came back approved.
