Anyone have BlueShield Access + HMO, gotten approved this year ..

or last year? I'm curious what they ask for, and what they will accept, as far as medically supervised diets are concerned. Any and all help about this insurance would be appreciated. I'm new to this insurance and i'm trying to find out everything I can. Thanks!
