Anyone have the LABPAND or ADJUSTABLE BAND covered by insurance?

Please email me if you or anyone you know have had the lab band (adjustable band) procedure done and it was covered by insurance. If so, any paperwork tips? I have Blue Cross HMO. Thank you. Sue
